The Illinois HIV Prevention Planning Group is requesting your input! Below is a link to a survey for Illinois men of age 18 or older living outside of the Chicago city limits who have been sexually intimate with other men. It will take only about four or five minutes to complete the survey. The survey is anonymous, and the information you provide to the Prevention Planning Group is crucial for planning good HIV prevention services for Illinois.
